Name of Product: Rage Wireless Guitars
Hazard: A circuit board defect can cause AA batteries used in the guitar to leak if the batteries are installed incorrectly, posing a risk of chemical burns to consumers.
Description: The Rage Wireless Guitar controller is designed to be used on the Nintendo Wii system. The guitar is blue or white and is 31 inches long. The guitar contains battery-operated LED-lighted fret buttons that go up the neck of the guitar.
Sold at: Mass merchandisers and specialty retailers nationwide from June 2008 through September 2008 for between $40 and $60.

Name of Product: Blue Ember Gas Grills
Hazard: The gas grills can be assembled improperly exposing the gas burner hoses to excessive heat, posing fire and burn hazards to consumers.
Description: Recall involves Blue Ember liquid propane (LP) or natural gas outdoor grills. The recalled model and serial numbers are listed below. The model and serial numbers are printed on a rating plate label on the rear of the grill. The cabinet style grill has two doors and is silver-colored and black or silver-colored and gray. "Blue Ember" is printed on the grill's hood.

Name of Product: "Gas Match" and "Ready, Aim...Fire" Multi-Purpose Lighters
Hazard: The multi-purpose lighters fail to meet federal safety standards because they might not contain a child-resistant mechanism, posing a potential fire and burn hazard to young children.
Description: The recalled "Gas Match" lighters have either a multi-colored red/white, blue/white or green/white plastic body, and a silver-colored metal nozzle. Each lighter measures 10½-inches long. The recalled "Ready, Aim...Fire" lighters have either a solid black, blue, red or green plastic body, and a silver-colored metal nozzle. Each lighter measures 9-inches long.
Sold at: Retail stores in New York State from August 2008 through October 2008 for about $1.

Name of Product: Shimano Dura Ace Carbon Clincher Wheels for Bicycles
Hazard: Rim surface and spoke hole plugs on the wheel can cause a puncture to the inner tube, resulting in a flat tire. This can cause the rider to lose control and fall.Incidents/Injuries: None reported.
Description: This recall involves the Shimano Dura Ace Carbon Clincher Wheel Sets with model number WH-7850 C24CL sold for road racing bicycles. The rim has labels that read "Shimano WH-7850", "Dura Ace", and "Carbon 1380." These wheels are intended for use with high end adult road racing bicycles.
Sold at: Bicycle specialty stores and dealers nationwide from April 2008 through August 2008 for about $1,300.

Name of Product: Rechargeable Batteries and Recharging Station for Didj Custom Gaming System
Hazard: The rechargeable batteries included with the Recharging Station can overheat if the gaming system is placed into the recharging base upside down, posing a burn hazard to consumers.
Description: The Recharging Station comes with two rechargeable batteries, an AC adapter, and a recharging base. The recharging base is white with a green indicator light on the bottom. The Recharging Station for Didj was sold as item number 30676, which is printed on the box and on the bottom of the recharging base.
Sold at: Department stores and toy stores nationwide and http://www.leapfrog.com/, and other online retailers from July 2008 through October 2008 for about $35.

Name of Product: Serta Zipper-Covered Foam Core Mattresses
Hazard: The mattresses fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers.
Description: The recalled mattresses have a zipper cover over a foam core. The mattresses were sold in twin, full, queen, king and California king sizes. They were manufactured between November 30, 2007 and May 20, 2008. The mattress name, date of manufacture, model, and serial numbers are located on label attached to the mattress. Only specific serial numbers are included in the recall. The recalled mattresses are listed in the chart below.

The U.S. Consumer Product Safety Commission (CPSC) and Carter’s, Inc., of Atlanta, Georgia, are advising parents and caregivers that they have received reports that a small percentage of babies and infants have developed rashes on the upper back after wearing Carter’s clothing with heat-transferred, or “tag-less,” labels.
This advisory applies to Carter’s Fall 2007 product line. The Fall 2007 line utilizes a label on the inside back of the garment that has a raised surface with a solid, rather than a stenciled, background. This advisory does not apply to previous and current product lines, which utilize labels with stenciled backgrounds.
The garments, which were made in various countries, were sold at Carter’s own retail stores and at department and national chain stores.
If your child develops a rash on the upper back after wearing garments that have a “tag-less” label with a solid background, you should stop using these garments. If the rash persists or worsens, you should contact your pediatrician.
